RP Dallas, a division of Singleton Associates, PA (“SAPA”), a Radiology Partners affiliated practice, has become aware of an unauthorized disclosure of your protected health information (“PHI”). We have thoroughly researched the incident, and while we have no reason to believe your personal information was used inappropriately, we want you to be aware of the incident.

What happened?

  • On 7/23/2024, SAPA discovered that during the period of October 2022 to August 2024 one or more billing statement/s intended for you was mailed to an incorrect guarantor address. This was due to incorrect information we received from the referring healthcare provider.

What types of information were involved?

  • The information on the document included your name, account balance, date of service, description of service, charge amount billed, payments and adjustments made, and insurance status.

Steps SAPA has taken to mitigate impact & prevent recurrence

  • SAPA has taken corrective measures to ensure that its systems have the correct address received from requesting healthcare providers, and that patient statements are mailed correctly.
  • These measures include improving system setup, updating intake procedures, and reviewing contract arrangements between SAPA and requesting healthcare providers.

Steps that you can take to protect yourself

  • Obtain a credit report from credit reporting agencies – Equifax, Experian, and TransUnion. The credit reporting bureaus provide consumers with a free credit report once every year if requested. If needed, you may place a fraud alert, security freeze, or a credit freeze on your credit files.
  • Monitor your accounts for any sign of suspicious or fraudulent activity.
  • In addition, you may contact the Federal Trade Commission (FTC) or law enforcement to report incidents of identity theft or obtain information about fraud alerts, security freezes, and preventing identity theft.

Given that this incident resulted in a breach under the Health Insurance Portability and Accountability Act (“HIPAA”), we will also be notifying the Office of Civil Rights as required by law.
